LEVELS OF CARE

We offer fixed fees with no hidden charges.

• Level One (low level of care): The resident needs occasional supervision, assistance or reminders to perform some daily activities, but is independent in other activities. He or she may require assistance or reminders to take medications, or may be able to take medications independently.

o 24 Hour Care Supervision by trained staff
o Monitoring and Safety Check o Minimal staff assistance for transfers and toileting o Nurse On-call
o Visiting Physicians/Home Care Nurse, PT and OT/Hospice Care available
o Weight/Nutritional Monitoring

• Level Two (moderate level of care): The resident requires more substantial support with some activities of daily living, while needing only minimal assistance with others. Assistance with medications may be provided.

o Moderate staff assistance with bathing, dressing, grooming with hands-on assistance
o Requires frequent verbal instructions and reminders for Activities of Daily Living, such as meals, medication, activities, etc.
o Incontinence Care such as toileting, diapering, and Foley catheter care
o Special Diet (pureed) with one-on-one assistance in feeding
o Memory Care (Alzheimer’s, Dementia Care)

• Level Three (high level of care): The resident needs frequent and comprehensive assistance with activities of daily living. Staff administers medications or assists the resident to take them; staff monitors the resident for the effects of medications.

o Staff provides total care with dressing, bathing, grooming, toileting and feeding o Bowel and bladder incontinence
o Wandering (attempts to walk out of home on frequent basis and awake at night most of the time)
o Sun downers Syndrome (becomes more confused and agitated and combative in late afternoon and evening)
o Wheelchair Dependent (requires lifting by staff or mechanical device (hoyer lift)
o Bed-bound (does not get out of bed)
